Name Changes
Your name as it appears on ACORN is the name that will appear on your diploma.
You may wish to update your legal name if you want to include your middle name, have recently had a legal name change or have special characters that you wish to be shown on your diploma.
If changes are not made prior to convocation, you will be required to order a replacement diploma.
If you want to add accents or special characters to your name or remove the period after an initial in your name, please complete the Accents on Diploma Name Form and return it to the Office of Convocation no later than April 23, 2025.
For all other name change requests, students must submit the Change of First Name or Gender Request Form along with valid (unexpired) Canadian government issued photo ID or passport to the UC Academic Advising & Registrar's Office. The completed form and a clear image or scan of the photo ID can be emailed to uc.registrar@utoronto.ca for review and processing, no later than April 23, 2025.
